Dark chocolate is the rare indulgence with real research behind it. The research is also narrower than the headlines, and most of what is sold as heart-healthy chocolate contains too little of the active compound to do what the studies did.
What is actually being studied
The compounds of interest are cocoa flavanols, particularly epicatechin. They appear to increase nitric oxide availability, which relaxes blood vessels — the same pathway as dietary nitrate, reached by a different route.
Trials of cocoa flavanols have found small blood pressure reductions, typically around 2 mmHg systolic, improved flow-mediated dilation — a measure of blood vessel function — modest improvements in insulin sensitivity, and small LDL and HDL changes.
Note that these trials generally use standardised cocoa flavanol extracts or specially prepared high-flavanol cocoa, not supermarket chocolate bars.
The largest trial, and what it found
COSMOS tested a cocoa flavanol supplement (500 mg daily) in over 21,000 older adults for around 3.6 years. On the primary outcome — total cardiovascular events — it did not reach statistical significance.
Secondary analyses showed a reduction in cardiovascular death, and a larger effect among participants who took the supplement consistently. Those are secondary and per-protocol findings, which are hypothesis-generating rather than conclusive.
The fair summary: the largest and best trial of cocoa flavanols missed its primary endpoint, with suggestive signals in secondary outcomes. Anyone citing COSMOS as proof that chocolate prevents heart disease is misrepresenting it.
The problem with actual chocolate
Several things stand between the trial data and a chocolate bar.
Flavanols taste bitter, and most commercial processing — especially Dutch processing (alkalisation), used to soften flavour and darken colour — destroys a large share of them. A dark bar may have far fewer flavanols than its cocoa percentage implies.
Cocoa percentage is not flavanol content, and flavanol content is essentially never on the label. Two 85% bars can differ several-fold.
Sugar and calories come along. An ounce of dark chocolate is roughly 150–170 calories.
Heavy metals. Testing has repeatedly found lead and cadmium in dark chocolate, with cadmium taken up from soil by the cacao plant and lead largely a contamination issue during drying. Levels vary widely by brand and origin. This is a genuine reason to keep intake moderate and vary brands, and it matters more for children and in pregnancy.
Milk chocolate and white chocolate do not belong in this conversation at all.
A reasonable position
If you enjoy dark chocolate, roughly an ounce (30 g) of 70%+ a few times a week is a pleasant habit with a plausible small benefit and manageable downsides. Choose bars that are not Dutch-processed where you can tell, and treat it as replacing a worse dessert rather than as an addition.
If you do not enjoy it, there is no health reason to start. The blood pressure effect is smaller than what you would get from a daily walk, better sleep, less sodium or more potassium (the DASH approach).
When to see a doctor
See a doctor if home blood pressure readings are consistently 130/80 or above — chocolate is not a treatment. Discuss cocoa or caffeine intake if you have reflux, migraine triggered by chocolate, or arrhythmia sensitive to stimulants. Chocolate is also toxic to dogs, which is worth remembering if bars live on a low shelf.
The honest bottom line
Cocoa flavanols have a real mechanism and consistently produce a small blood pressure improvement in trials — but the largest trial missed its primary endpoint, and the flavanols studied are largely destroyed by the processing that makes chocolate taste good. Enjoy an ounce of a good dark bar for what it is. It is not medicine.
