Exercise is one of the most powerful health interventions available. It is also, for weight loss specifically, far weaker than almost everyone expects — and understanding why prevents a great deal of frustration and self-blame.

The arithmetic is discouraging on its own

A brisk 30-minute walk burns something in the region of 150 to 200 calories for an average adult. A blueberry muffin contains roughly 400. A large latte and a cookie can exceed an hour of running.

This is not a moral point about willpower. It is a statement about the relative size of two numbers. It is far easier to consume 500 calories than to burn them, and any plan that relies on out-burning intake starts at a disadvantage.

Compensation makes it worse

If exercise merely added to your daily burn, the arithmetic would at least be predictable. It does not, because the body responds in two ways.

Appetite rises. Not always, not in everyone, and with wide individual variation — but on average, increased energy expenditure produces increased intake. Some people are strong compensators and some barely compensate at all, which is part of why exercise-and-weight studies produce such variable individual results.

Unconscious movement falls. This one surprises people. After a hard training session, people tend to sit more, fidget less, take the lift, and generally move less for the rest of the day. That reduction in non-exercise activity can quietly cancel a meaningful share of what the workout burned.

The constrained energy model

There is a more provocative idea with real evidence behind it. Research comparing highly active populations — including hunter-gatherer groups whose daily physical activity vastly exceeds that of office workers — found their total daily energy expenditure was not proportionally higher than that of sedentary Westerners once body size was accounted for.

The interpretation, known as the constrained total energy expenditure model, is that the body adapts to sustained high activity by reducing energy spent elsewhere — on inflammation, stress response, and other background processes — keeping total expenditure within a narrower band than simple addition predicts.

This remains debated among researchers and should not be treated as settled. But it is consistent with the repeated clinical observation that exercise interventions produce less weight loss than the calorie arithmetic predicts.

So why exercise at all?

Because weight is a poor measure of what exercise does. If you judge it by the scale alone, you will conclude it failed while it is doing most of its work elsewhere.

Cardiorespiratory fitness is among the strongest predictors of mortality there is, and it is largely independent of weight (the evidence).

It lowers blood pressure reliably (the protocol) and improves insulin sensitivity through a pathway that bypasses insulin entirely (how).

It preserves muscle during weight loss, which determines whether what you lose is fat and whether it stays off (why muscle decides).

It is the single best predictor of keeping weight off. This is the crucial asymmetry: exercise is weak for losing weight and strong for maintaining the loss. People who successfully maintain are consistently more physically active than those who regain.

And it improves mood, sleep, bone density and cognition — none of which appear on a scale.

What to do with this

Do not eat back “exercise calories”. Fitness trackers and cardio machines overestimate burn substantially, and eating back what they report frequently erases the deficit entirely.

Prioritise strength training over extra cardio if the goal is body composition.

Raise everyday movement rather than only adding formal sessions — it is the most variable component of daily expenditure and the least compensated (how many steps).

Time some of it after meals. A short post-meal walk does disproportionate work for blood sugar even if it does little for the scale (the trial).

When to see a doctor

Most healthy adults can start moderate activity such as walking without clearance. Check first if you have known heart disease, have had chest pain or fainting with exertion, have poorly controlled blood pressure or diabetes, or plan to start vigorous exercise from a sedentary baseline. Stop and seek care for chest pain, unusual breathlessness, dizziness or an irregular heartbeat during activity.

The honest bottom line

You cannot outrun a bad diet, and the reason is not weak willpower — it is arithmetic plus compensation. Appetite rises, unconscious movement falls, and total energy expenditure may be more constrained than simple addition suggests. Create the deficit with food, and use exercise for what it is genuinely excellent at: fitness, blood pressure, insulin sensitivity, muscle, mood, and keeping the weight off once it is gone.
